USA Field Hockey Insurance is a SEPARATE FEE and must be paid by the player directly to USA Field Hockey. It is NOT included in the registration cost. Please visit: http://usafieldhockey.com/  (2023 insurance costs are: U-12 $25.00 or U-19 $60.00) 
Players will need to print proof of membership and submit to coaches at the start of practices.

Players are also responsible for their own Stick, Mouth Guard and Shin Guards.
